Moderna and Pfizer bivalent now available.
Moderna is for 18 years and older. Pfizer is for 12 years and older.
Prerequisites: Patient must have completed primary series (First and Second Dose) AND must be 2 months since last Covid-19 Vaccination.
Please bring the following to your appointment (even if under the age of 18):
- Identification card (ID, Passport, Birth Certificate, or School ID)
- Insurance Card
